 Welcome to Yavapai County Long Term Care's (YCLTC)  Provider Website.

• The service provided for you on this web site will enable you to search on-line medical records (Eligibility, Enrollment, Prior Authorizations, Submit Authorization & Claim Status) for YCLTC.
  In order to use the on-line service you must have an active account (User name and password).
  All YCLTC Providers are eligible to create an account and use this service.

• Accounts are assigned by YCLTC. To request an account, please fill out the Account Request form and mail it or fax it to:

  When the account set up process is complete, a letter will be sent to the mailing address specified by the the account holder.
